Position Summary

This role is hybrid with requirement to be commutable to one of the following offices: High Point, NC, Chandler, AZ, or New Albany, OH. Ability to go to the office for meetings, trainings, etc is required.Standard business hours Monday-Friday.What is A1A?A1A is a fully integrated population health and customer service solution for large plan sponsors. Our embedded and customer-dedicated service, clinical, and rising risk pods allow maximization of inbound and outbound touchpoints to solve members’ needs and create behavior change. Our white-glove service and end-to-end ownership of member support creates a trusted partner in health. We are the premiere service and clinical offering for Aetna nation-wide and create industry-leading solutions for our customers and members.

This role partners with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member’s benefit plan and/or health needs. Responsibilities include researching available options and resources for members with financial, transportation, safety, and housing needs. Population spans the entire lifecycle. All member contact is done telephonically or via electronic exchange. This position works to empower members to make positive lifestyle and medical choices and works as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health. Requires an LSW or MSW with unrestricted active license.

Required Qualifications

  • Must have active current and unrestricted LSW/MSW, LISW or LMSW license in state of residence.

  • LMSW will be required to obtain 10 additional state licensure before new hire training is completed.

  • 2+ years experience in medical social work or case management.

  • Must be able to work Monday through Friday 8:00am to 4:30pm EST/Arizona Time

    • Must reside within 50 miles of one of three locations: High Point, NC, Chandler, AZ, or New Albany, OH.
  • Must be able to travel to office location up to four times per month for required Meetings/panel

    • Proficiency with computers and an ability to navigate multiple systems simultaneouslyPreferred Qualifications

    • Telephonic Social Worker experience

    • Advanced care planning experience

    • Hospice/palliative care experience

    • Case management and discharge planning experience

    • Managed care/utilization review experience

    • Crisis intervention skills

    • Ability to exercise independent and sound judgment in decision making

    • Highly organized to manage continuously changing priorities

Pay Range

The typical pay range for this role is:

$46,597.20 - $101,200.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
