Job Description: Nurse Practitioner/Physicians assistant:

• Assessment and Diagnostic Work-Up: Conduct thorough patient assessments, including health history, physical examinations, and diagnostic evaluations.

• Clinic Intervention: Collaborate with the Primary Care team to deliver patient education and interventions.

• Ordering and Evaluating Diagnostic Tests: Determine appropriate diagnostic tests, interpret results, and recommend treatment or referrals.

• Health Record Documentation: Utilize the electronic comprehensive patient record system (CPRS) to record health appraisals, clinical assessments, treatment decisions, and ongoing management.

• Medication Management: Order medications as needed for patients.

• Referral Consultations: Facilitate referrals to specialists when necessary.

• Patient-Centered Care: Prioritize patient well-being and ensure timely documentation on the day of each encounter.
